Late-onset Endophthalmitis Caused by Streptococcus Cristatus after Trabeculectomy

Purpose: To report a case of late-onset bleb-associated endophthalmitis caused by <i>streptococcus cristatus</i> (<i>S. cristatus</i>) after trabeculectomy.Case summary: A 59-year-old woman presented with left ocular pain and visual disturbance. She had undergone trabeculectomy the eye 2 years ago. At initial visit, she was only able to count fingers from eye. Whitish thin bleb, conjunctival injection, inflammation were observed in anterior chamber, fundus could not be visualized. Despite administering topical systemic antibiotics, chamber reaction worsened, hypopyon inflammatory membrane noticed next day. Vitrectomy phacoemulsification intravitreal antibiotic injection performed, injections repeated over two days. Bacterial culture vitreous fluid showed growth <i>S. 5 days surgery, improved 7 One month later, still eye, but there no signs endophthalmitis.Conclusions: Postoperative glaucoma filtering surgeries has been reported previously Korea. best our knowledge, this is first Korean cristatus</i>. If gram-positive cocci are grown samples patients trabeculectomy, resident oral microflora such as should considered causative pathogen.
